Payment is due in full at the time of service. Forms of payment accepted: Cash, Check, Credit/Debit Card, HSA, Venmo, Zelle

A credit card with be kept on file with our office for your convenience.

If you must cancel an appointment, we kindly request that you give us at least 24 business hours notice by phone or email to avoid a $50.00 no-show fee (exceptions may be made for emergencies/sudden illness).
